What is the address of the new practice?

The new practice is located at 2122 West Cypress Creek Rd. Suite 200, Fort Lauderdale, Florida 33309.

What if I have an emergency?

Please use emergency services, an Emergency Room or 911 if necessary. 

 

Will the new practice take my insurance?

Most likely. Dr. Caldwell will be signing up with insurance plans in his new practice. However, many plans have a variable time-frame with this process. He hopes to be on the same plans as before.  In some cases, patients may prefer to see Dr. Caldwell “out-of-network” to fulfill their orthopedic needs. Please call us to find out.
